For patients facing aortic valve replacement, the choice between transcatheter aortic valve replacement (TAVR) and surgical aortic valve replacement (SAVR) is rarely simple. In a new joint statement, cardiologists and heart surgeons stressed that each decision involves careful, individualized evaluation rather than a one-size-fits-all approach.
TAVR, a minimally invasive procedure, has grown in popularity over the past decade. It allows doctors to insert a new valve through a catheter, often without stopping the heart. SAVR, the traditional open-heart surgery, requires a sternotomy and a longer recovery. But the statement makes clear that newer does not always mean better.
Clinicians now weigh multiple factors beyond age and surgical risk. These include the patient's overall anatomy, the shape and size of the aortic root, the presence of other heart conditions, and the long-term durability of the valve. For younger patients, SAVR may still offer a more proven track record over decades. For older or frail patients, TAVR often provides a faster recovery with fewer immediate complications.
The statement also highlights the importance of shared decision-making. Patients need to understand the trade-offs: TAVR avoids a large incision but may carry higher rates of permanent pacemaker implantation or paravalvular leak. SAVR is more invasive but has a longer history of reliable outcomes.
